This mound was failing due to an organic overload caused by chemicals being added to the system see slide below
This photo depicts the sludge level in the pump tank serving the mounds above. This system was only 2 years old at the time of failure. The sludge levels present in this photo would not normally be present. An analysis of the system indicated that chemical addition by the user was the ultimate cause of the failure

The mound above failed due to both poor design and construction
